Authorization for background check

"Authorization for background check" means a complete, signed form prescribed by the Department which authorizes a background check and submission of fingerprints, if required. An authorization for a background check may be used for the initial and all subsequent background checks required to determine compliance with the requirements of this Part. (R 385)
